I hereby state that my child has had a recent physical examination and is it satisfactory condition to participate in dance or gymnastics, and he/she has no physical conditions. By giving my child permission to participate in this program, I acknowledge that any activity involving height and motion (such as dance and gymnastics) involves the risk of injury, ranging from minor injuries (such as bruises and sprains) to serious or even catastrophic injuries (such as permanent paralysis or even death). I hereby release Rose Tree Dance Center, their officers, owners, employees, sub-contractors and teachers from any and all claims for damage to persons and property which might arise as a result of an accident occurring while my child is participating in the program, including class participation, shows, competitions, and transportation to such shows and competitions. I hereby state that I have read and understood the above release.

Tuition Policy Payment Schedule There are 5 payments due each year. Our year runs from September through the recital in June. Payments are due: September 1st Payment #1 November 1st Payment #2 January 1st Payment #3 March 1st Payment #4 May 1st Payment #5 Regular Rate & Discounted Rate To receive the discounted rate full payment must be received no later than the due date.
There is no additional charge for more than 5 class Pre-payment option REDUCE YOUR TUITION EVEN MORE! Whether you choose payments or session you can reduce your tuition an additional 10% by remitting your payment 30 days or more in advance of the due date.
Example: Your discounted payment rate is $90. You send your September payments to us 30 days in advance and deduct 10%. Your payment #1 is now $81.00 instead of $90.00. If at any time you wish to discontinue our service, further billing will end upon the written confirmation that you wish to discontinue classes. Tuition payments are not refundable or transferable. Session Payment 2 Payments per year. You can save 10% of your tuition by making your payment by session. Session #1 runs from September through January. Session #2 begins February 1 and runs through the recital on June. Session payments are not refundable or transferable.
